Employee Onboarding Forms

Complete the following forms prior to starting work:
** ALL IDENTIFICATION DOCUMENTS (AS DESCRIBE IN I-9 FORM PAGE 2) MUST BE BROUGHT ON YOUR FIRST DAY OF WORK.**  Email all completed forms to admin@paintblastus.com. If you do not have a computer or printer at home please let us know and we will have the foreman or project manager provide you with the forms on your first day of work. â€‹â€‹

Incident Report Forms

Incident Reports help us identify potential hazards, implement corrective measures, and prevent future incidents. Accurate incident reporting is crucial for compliance with regulatory requirements and protecting both our employees and the company from potential liabilities.

What tools am I required to bring?

All foreman and painters are requires to come to the job site with the tools listed below.

If an employee arrives on site without the required tools and safety equipment, then they may be asked to leave the job site to retrieve these items. In this case the employee will be asked to clock out and only clock back in when they return with all their items in hand. Paint Blast US has some extra tools at our storage location which can be purchased by employees at request. Expenses will be deducted from their next paycheck.

  • Paint Brushes in various sizes: Ranging from 2” to 4” (All brushes must be maintained in good condition.)

  • Roller Frame, Roller Pole

  • Putty Knife

  • Cutting knife

  • Caulking Gun

  • Wire brush

  • Painter’s Multi Tool: 5 in 1

  • White Painter’s pants

  • Hard Hat

  • Safety Vest

  • Construction boots

  • Respirator (PPE)

  • Hand Masker

  • Phillips & Standard Screwdriver

  • Hammer

  • 4’ ladder (Preferred but not required for apprentices.)

Travel Requirements
All employees are required to have their own mode of transportation to and from work.​
​
​High-visibility clothing (such as a reflective vest or a neon shirt) to increase visibility and safety.
  1. Safety Footwear: Closed-toe shoes or boots with slip-resistant soles to prevent injuries from falling objects and slipping on wet or painted surfaces. NO sandals, flip-flops, or other open-toe shoes that do not provide adequate protection.

  2. Safety glasses or goggles to protect the eyes from splattering paint, dust, and debris.

  3. Wearing a dust mask or respirator to protect against inhalation of paint fumes, dust, and other airborne particles.

  4. Head Protection: A hard hat should be worn at all times when you are on site.

  5. Weather-appropriate Clothing:

    • Dress in layers to adapt to changing weather conditions. Consider wearing a jacket or rain gear if working outdoors or in areas exposed to the elements.

  6. High-visibility Clothing:

    • Wear high-visibility clothing (such as a reflective vest or a neon shirt) to increase visibility and safety.

  7. Personal Protective Equipment (PPE):

    • Depending on the specific tasks and hazards involved, additional PPE such as hearing protection, knee pads, or fall protection equipment may be required. Speak to the foreman or ask the project manager what might be required BEFORE showing up on site.

Employee Handbook
A comprehensive document that outlines the policies, procedures, and expectations of a company or organization. Hazard Communication Program

A written program addressing labeling and warning requirements, material safety data sheets (MSDSs) and employee training on hazardous materials. Foreman and project managers to maintain maintain a list of all hazardous substances in the workplace and a description of the methods the employer will use to inform employees of the hazards related to nonroutine tasks in the workplace.

 

Fall Protection Program
